Printed envelope-style policy wrapper completed in ink, about 3.8 x 8.3 inches. Used condition: even toning, edge wear and chipping, a loss at one lower corner, and soiling on the back.

Amount $500, cash $0.35, page 392, agent C. H. Yoder, with Smith Orr, Secretary, Apple Creek, O. printed at the foot. A brown 2-cent stamp is affixed on the front and pen-cancelled.
